Please read each step carefully prior to completing each form or item of paperwork. All of the paperwork is carefully
reviewed by a League official. Paperwork will be easily rejected if it does not meet the specifications.
Your child's first and last name must be consistent throughout all of the paperwork. For example, if his first name is
Robert, use either Robert or Bobby throughout. Do not use both forms of the name.
|
|
| 2. |
Locate and make a photocopy of your child's birth certificate.
|
|
| 3. |
Locate or cut-out a 1" x 1", high-quality, color photograph of a head-shot of your child. Write your child's
full name on the back of the photo. The photo will be used on your child's state player pass. Note that the league
will reject any low-quality, home-printed photos. Please do not bring us larger photos that need to be cut out; cut
them to size yourself.
|
|
| 4. |
Use a paper clip to secure the photo to the front of the birth certificate and set them aside for now.
Do not use staples on any of the paperwork.

Print and fill-out the Medical Release Form.
Do not sign and date this form until you are with a Notary Public. Forms that do not have consistent dates will be rejected by the League.
We hope to have a notary public present at the Registration Night. Forms can also usually be notarized at your personal bank by a notary.

| 8. |

There are four slots for signatures at the bottom of the form. Be sure that you and your child both sign the form, leaving two slots empty.

- 1" by 1" color, high-quality head-shot photo (photo paper only)
- Coach Membership Form.

Review all paperwork for correctness and completeness. Missing or incorrect information can result in your child
being unable to play in a pre-season tournament, or not being registered in time for the first game. Then use a
paper clip (remember, no staples) to secure all completed forms together with the photo and birth certificate on
top.
|
|
| 13. |
Bring the packet of forms with you when you come to registration night. Remember to also bring your
checkbook to registration night. Your child will not be registered with the league until we receive
both the registration and unifo9rm checks.
|
|
| 14. |
If you cannot make either the Registration Night or the two office hour nights at the Registration director's
house (along with your child), deliver the packet to your head coach or team manager so that they can submit them.
Please note that you will have to arrange a time to have your child sign the player pass with your coach and/or team manager.
