Read through all the remaining steps before undertaking any of the steps below.

Locate and make a photocopy of your child's birth certificate.

Locate or cut-out a 1" x 1", high-quality, color photograph of a head-shot of your child. The photo will be
used on your child's state player pass. Note that the league will reject any low-quality, home-printed photos.
Please do not bring us larger photos that need to be cut out; cut them to size yourself.

Use a paper clip to secure the photo to the front of the birth certificate and set them aside for now.
Do not use staples on any of the paperwork.

The following forms are in Adobe Acrobat (.pdf) format. In order to open and print the forms, you must have a recent
copy of Adobe Acrobat on your machine. If you do not already have Adobe Acrobat installed, you can get this free
software by clicking the following icon:

Print and fill-out the Registration and Medical Treatment Authorization Form.
The club name should read: "Metuchen Travel Soccer Club".

If you are not a coach, assistant coach or team manager, skip to the last step.
|

If you are a team's head-coach, assistant coach or team manager, print and fill-out a
MTSC Secure Player Contract Form.

Review all paperwork for correctness and completeness. Missing or incorrect information can result in your child
being unable to play in a pre-season tournament, or not being registered in time for the first game. Then use a
paper clip (remember, no staples) to secure all completed forms together with the photo and birth certificate on
top.

Deliver the packet to your team's coach or team-manager as soon as possible. You should give the coach your
registration and uniform checks along with your paperwork. Please leave your coach time to review the paperwork
and get back to you with any errors or omissions.
