Read through all the remaining steps before undertaking any of the steps below.

Locate and make a photocopy of your child's birth certificate.

Locate or cut-out a 1" x 1", high-quality, color photograph of a head-shot of your child. The photo will be
used on your child's state player pass. Note that the league will reject any low-quality, home-printed photos.
Please do not bring us larger photos that need to be cut out; cut them to size yourself.

Use a paper clip to secure the photo to the front of the birth certificate and set them aside for now.
Do not use staples on any of the paperwork.

The following forms are in Adobe Acrobat (.pdf) format. In order to open and print the forms, you must have a recent
copy of Adobe Acrobat on your machine. If you do not already have Adobe Acrobat installed, you can get this free
software by clicking the following icon:

| 6. |
Print and fill-out the Player Membership Form.
Leave the following fields blank: League, League #, Club, Club #, Team #, Player Pass # and Age. We will fill those
in for you. Be sure to fill out the bottom section where you print and sign your names and provide the date signed.

ADDS and TRANSFERS ONLY
Only perform this step if you are being added or transferred to a team after regular season registration has closed. If you are preparing paperwork for
the regular season registration (which occurs in August for most teams), then you do not have to perform this step.
Print and fill-out only the fields specified below on the
Player/Coach Add or Transfer Form.
Only fill out these fields: Name, Date of Birth, Gender, Added Player/Coach signature and Date. Leave the rest blank and contact the club's
director of registration to arrange an appointment to have a player/coach pass filled out and to help you complete this form.
When you have completed this form with the registration director, and only after he/she fills in the missing information,
you will need to make 3 additional copies before delivering the paperwork to the league.

Print and fill-out the Medical Release Form.
This form must be notarized. We will provide a notary at both registration nights. However, if you will not be at either
registration night, you must take care of getting the form notarized yourself before handing in your paperwork to your child's coach.

Print and fill-out the S.A.G.E. Form
There are four slots for signatures at the bottom of the form. Be sure that you and your child both sign the form, leaving two slots empty.

If you are not a coach, assistant coach or team manager, skip to the last step.
|

| 10. |
If you are a team's head-coach, assistant coach or team manager, print and fill-out a
KidSafe Disclosure Form.
Also, if you are new to coaching in Metuchen Travel Soccer Club, print and fill out a
MTSC Secure Player Contract Form.

If you are a team's head-coach or assistant coach, make a photocopy of your state coaching license.  You
must also print and fill out a Coach Membership Form.

Review all paperwork for correctness and completeness. Missing or incorrect information can result in your child
being unable to play in a pre-season tournament, or not being registered in time for the first game. Then use a
paper clip (remember, no staples) to secure all completed forms together with the photo and birth certificate on
top.

If you are going to one of the registration nights, set the packet aside for now. Bring the packet of forms with
you when you come to registration night. Remember to also bring your checkbook to registration night. Your
child will not be registered with the league until we receive both the registration and uniform checks.

If you cannot make either registration night, deliver the packet to your team's coach or team-manager as soon as
possible.  You should give the coach your registration and uniform checks along with your paperwork.
Please leave your coach time to review the paperwork and get back to you with any errors or omissions.
